Our friendly and welcoming hospices in Bradford and Newcastle support people living with any terminal illness or approaching the end of life, and the people close to them.
Our nurses, healthcare assistants and other healthcare professionals in the North East and Yorkshire bring the clinical, practical and emotional help you need to you, in the comfort of your own home. And we offer support to the people close to you too – from reassurance and practical information to letting them take a break.
In the North East, the Hospice Care at Home service is available in County Durham, Middlesbrough, North Tyneside, Newcastle and Gateshead, South Tyneside and Sunderland.
In Yorkshire, the service is available in Calderdale, East Riding of Yorkshire, Kirklees, North Yorkshire, Vale of York, Wakefield and Bradford.
Whether you're ill or looking after someone who is, you know that symptoms can change at any time, day or night. Terminal illness doesn't adhere to a schedule, so neither should the care you get. Urgent hospice care at home is there for you when you need it.
Our Urgent Hospice Care at Home service is available in Calderdale in Yorkshire.
Companion volunteers in the North East and Yorkshire provide practical and emotional support at home and over the phone. They offer free, flexible and personalised help to people at the end of life, and to their families and carers.
The Companion at Home service is available in Tyne and Wear, Durham and West Yorkshire.
Our REACT service in Bradford helps people at the end of life avoid being admitted into hospital unnecessarily by working with the hospital emergency department to provide a specialist palliative care assessment. If your needs can be met by our multi-disciplinary team, we can discharge you back to your home where you will be supported there, with onward support from your GP and community nursing teams if needed.
This service is based in the Emergency Department at the Bradford Royal Infirmary and in the heart of the Bradford community.
